Newbie configuration problems

I have recently installed MV 3.11 and am having problems getting even
the demo running.  The daemon is running fine (FreeBSD 3.0 if it
matters).  I've looked over the documentation and FAQ.

I believe that it is configuration mistakes on my part. The Apache
(Stronghold 2.4.1) server reports premature end of script headers.

Here is an entry from the MV error log:

myaddress - - [11/February/1999:10:19:45 -0500] -
/cgi-bin/simple/scan/mp=0/se=Renaissance Undefined catalog:
/cgi-bin/simple

There is a file named /cgi-bin/simple, but I expect that the error
refers to something other than the filename.  Any pointers on how to
track this down?
