I'm having a similar problem installing minivend on Win98 with Personal Web Server. Personal web-server appears to be experiencing difficulty routing requests to the minivend document directory through simple.exe. On Win95 using personal web-server, I've gotten errors about redirection through ports when I use other programs that redirect via an executable.
I know this won't help fix the problem, but it may assist in reaching a solution.
