how to calculate shipping tax?

How can I calculate shipping tax based on the country and the state?  The
current system and documents only describe a state-based calculation.
However, we sell in over 100 countries...  When I type in a country of
Albania and a state of CA, I get California sales tax applied.  Uh, that
isn't exactly what I wanted.  For all I know, Albania might have a region
with an abreviation of CA.  I think there is an abreviation for a state in
Brazil that is CA.

How do I get sales tax to trigger only when the country and the state
match the criteria?

(MV 3.12, Perl 5.005, Red Hat Linux 6.0)

I looked through the docs and the mailing list for references to sales tax
and found several, but nothing that I understood as aplying to my problem.

Any help is appreciated.  I can't imagine that I am the first to run into
this problem.
