I had this problem or at least one similar to it.  I messed up the way
that I was telling MV what action to perform.  Look at the simple demo
very carefully.  The update button on the checkout page does not do the
same thing as the button that goes to the receipt page.  look carefully at
the use of mv_doit, mv_action, mv_click etc.  I don't remember what I had
wrong, but it was one of these and suddenly I got the symptoms you are
describing.
