what do you mean about the credit card number?
Did you remove the information about this credit card number from your
checkout page?

Regarding the MV_SAMPLEURL and MV_SAMPLEHTML. If your real URL to your
Site is e.g. "http://blobbie.wino.com",

Variable    SAMPLEURL   blobbie.wino.com

If your real URL to your Site is e.g.
"http://blobbie.wino.com/winesite",

Variable    SAMPLEURL   blobbie.wino.com/winesite

If your directory is "/home/httpd/html/winesite"

Variable    SAMPLEHTML    /home/httpd/html/winesite

Once more:
If visitor has to see your shop at "http://blobbie.wino.com", 

Variable    SAMPLEURL   blobbie.wino.com

else if "http://blobbie.wino.com/winesite"

Variable    SAMPLEURL   blobbie.wino.com/winesite
