International Shipping - call for pricing

Ok, everything is ALMOST working :)  I solved the bug with the checkout 
page, my .html file didn't have secure=1 on all the forms in it for some 
reason *shrug*

Anyway, last big hurdle is making shipping work.  My client charges fixed 
shipping costs for most products if shipped domestically.  I read the docs 
and that part looks easy.  The part that doesn't look so easy is they want 
international customers to get told that the shipping charges have to be 
calculated inhouse.  Perfect world scenario would be that the customer 
places the order online, shop costs out the shipping and contacts the 
customer, and if he's still interested, the customer can go online and 
retrieve his saved order which now has the shipping field all filled in and 
input his credit card.  Of course, I have NO idea how to implement anything 
cool like that :P

How do you deal with shipping costs that can't be calculated thru Minivend 
for whatever reason?
