[mv] Complicated shipping problem (for me)

I have a really complicated shipping situation that I'm having a hard time
dealing with. I was wondering if anyone has any ideas on the best
approach.
Here's how it's supposed to work:
Orders to WA, OR, ID, CA, UT, & NV: Ground 15% of total, $5.00 min.
Orders to OR, ID, CA, UT, & NV: 2day air 20% of total, $10.00 min.
Orders to all states except AK: 2day air 30% of total, $10.00 min.
Orders to AK: 2day air 35% of total, $18.00 min.
Orders to British Columbia, Canada: Ground 35% of total, $12.00 min.
Orders to other parts of Canada: Ground 40% of total, $15.00 min.
Orders to Hawaii are not to be allowed, and any order to Canada
requires a Phyto Sanitary Certificate is to the tune of $35 per
$300.00 in the order.
The problem that I'm having is in hiding shipping options that aren't
available for a given state. For example, WA only has ground shipping
as an option. For now I've been setting a really high multiplier in the
hopes that someone won't place an order with $1,000,000 in shipping, but
it's a hack.
I also am not sure how to deal with the no orders to HI bit either. I've
been dealing with it in the same way -- high multipliers. I know that
I can use the 'e' shipping calculation mode to display a message, but
that doesn't prevent someone from placing an order with $0.00 in shipping.
Is there a way to only display certain menus for certain states? Will
I have to set up a state database similar to country.asc?
But the biggest problem that I can't solve is the automatic $35
surcharge/$300 to Canada. What's the best way to deal with that?
I really appreciate any help, and if it's of any interest I'll post
a summary of the final procedure.
