[mv] Credit card checkout: emails work, but HTML receipt fails

Dear Minvend gurus,

I am running the latest Minivend version on RedHat Linux 6.1.
When I checkout with credit card validation (with or without SSL), 
the email receipt made by my customized ./etc/receipt.html and the order 
made by ./etc/report, work fine. However, the HTML page built by
./etc/receipt.html does not display to the browser. Instead the user gets:

   500 Internal Server Error
   The server encountered an internal error or misconfiguration and 
   was unable to complete your request.

The mysterious problem is that there is no telling trace in the httpd (Apache)
error_log, or in Minivend's error.log for the cause of this error.
I also ran 'expireall' and of course restarted the minivend server dozens
of times with many tweaks, but no luck.

Some clues:

0) As noted above, results that use sendmail are OK. Also, the PGP-encrypted
card data are attached to the order email (from ./etc/report).

1) tracking.asc also has a record of the credit card orders.

2) ./etc/receipt.html gets properly displayed to the browser and (emails are
sent), 
when I use 'fax_order=1' (Call order in). But, when I use
mv_credit_card_number=1, 
even with mv_credit_card_force=1, I get the above error from the Web server.
I am using 'config CreditCardAuto', not 'config CyberCash'.

3) A search of the email archives shows only this message posted in 
November 1999 that was very similar to my problem:

However, the solution was not posted as far as I can tell.

I will very much appreciate any suggestions you can offer, and will
post the final solution when I find it.
