[mv] bad order problem

Hello,

I've had a few orders that I know about that I never received.  The user
placed the orders and got a confirmation email (sent on receipt.html) but
the order never made it to report.html where its logged to a sql database
and email sent.

Nothing shows up in the tracking or backend .asc files either.

One clue:  the receipt the customer gets does NOT have a mv_order_number on
it.

Any ideas?  Any way to track the orders that I don't have any tracking for ?
