[mv] Problem changing default index.html in version 4.00

I am having a difficult time changing the default index.html page in the
simple demo.
I had an index page setup using version 3.14 which split into two frames
(one of which split into 2 more frames).
I am trying to do the same thing, with version 4.00. After running makecat,
my makecat.cfg file contains this section:

samplehtml=/home/minivend/swtest

servername=192.168.1.111
catroot=/usr/lib/minivend/catalogs/swtest
The catalog which was produced appears to contain the index.html template
which was in /usr/lib/minivend/simple/html/index.html
When I overwrite this index.html with a different one, and enter the url:
//192.168.1.111/cgi-bin/swtest
the old index.html page is outputted.
I changed the default index.html page in /usr/lib/minivend/simple/html/ ,
reran makecat and this resulted in the new index.html page
being produced in /home/minivend/swtest.
However, the old index.html page still appears when I enter that url.
Any ideas?
