[mv] Shipping in 4.04

Hello, I'm running minivend 4.04 and am having a problem with shipping. I
used the simple template, no multi-order pages, however shipping worked
once and has since quit working. All it displays is $0.00.
The shipping structure is everal price breaks with a fixed shipping
amount, and then if over $200, its 6 percent.
Here is my shipping.txt file

However nothing ever shows up in shipping. Does anyone have any
suggestions on what to try next?
I changed all the referneces I could figure out how to change
(mv_shipmode) and so on on the checkout.html to the standard type
as well as changing it on the country.txt for the US code.
