[mv] curious problem with login.html in 4.04's simple demo

I'm experiencing a curious issue with a 'simple' catalog I'm converting
for a client. Minivend is looking for the wrong checkout page, and I
can't figure out why.
Here's my code I use on that page:

However, I keep getting sent to this error when I hit submit:
Sorry, the page (ord/checkout) was not found
What possibilities exist for me to have mv_nextpage set to
"multi/checkout", yet having minivend going to look for "ord/checkout"?
I grepped through the catalog to see if I could find where this might be
hardcoded. 'ord/checkout' appears to be hardcoded on this page of the
simple demo:
pages/special/needfield.html
but I don't think that is related.
In my catalog.cfg, I have this set:
Variable ORDER_STYLE multi
As well as this:
SpecialPage checkout multi/checkout
