[mv] Simple catalog install doesn't show info at top after I select something

I installed MV 4.04 on Apache with Perl 5.005_03.

It went well but when I go to the store (www.defcon-systems.com/simple2) and
select any of the buttons such as product or the "Enter" link, the 'frame'
around the main window disappears.  It displays the correct info but not the
top banner or the buttons on the bottom.
