[mv] Add Item At Checkout

I have my checkout as 2 pages (checkout and checkout2).

on checkout page 1 I want to offer the option of buying a membership card.

I setup this.

works farely well EXCEPT, if they unclick the box, then the item 
isn't removed :/. if you are on checkout 2 and go back to checkout 
page one and resubmit info on page 1 the item is added yet again.
So I need it, if clicked to add ONE card to the order, and if it's 
unclicked, it needs to be removed.

The other problem I am having is.
The customer wants shipping setup based on number of items in the basket.
$4.00 plus $1.00 PER item. But doesn't want the membership card to 
count as a item (aka doesn't want them charged $1.00). BUT if they 
order over $50.00 in merch, they want free shipping :)

P.S. how come everytime I use search on minivend I always seem to get
Bad Search Specification

The search could not be done. The problem was:
