[mv] MV3.14-5: using mv_order_item on user entered SKU

I have a situation that has arisen where I must automatically apply a
setup charge to special product in our distributor ordering system.
Adding the extra field to the products.asc file isn't a big deal, but
I've looked through the docs and archives at the use of MV_ORDER_ITEM
and I haven't determined a way to use it for our system.

Unlike the examples listed, which consist of a URL to click on, the
distributor enters the SKU number and quantity and the item is then
added to the basket.

Assuming that in products I create a field, say setup, which contains
the SKU for the setup charge on the current row, how to I get MV to
perform a conditional add of this second SKU?

Any suggestions would be appreciated.
