Works for me.
mkdir pages/test_access
touch pages/test_access/.access
cp pages/index.html pages/test_access
echo '*: [if cgi allowed]1[/if]' > pages/test_access/.access_gate
At that point, I get access with the URL
http://MACHINE/cgi-bin/simple/test_access/index.html?allowed=1
but not otherwise.
Perhaps some symptom description would help.
