[ic] adding fields to final html

I want to add some fields to the final.html page, like the add me to mail list and request an email copy. I would like to add things like a drop down of where you heard of us from. 
I can not find the file that gets e-mailed to me when the transaction is finished, and I didn't know if I had to add any fields to any other tables to do this. If someone could let me know what has to be done to complete this, I would appreciate it.
