Stuart -

Probably the best thing for you to do is to set fax_order=1 permenantly by 
using a hidden input on your checkout page:

<input type=submit name=fax_order value=1>

Then you can remove [value mv_payment] from the receipt and report pages so 
there is no message regarding payment type.

Finally set CreditCardAuto to no in your catalog.cfg and reconfigure.
