Problem with Personal Information Not Clearing

When I go through the process of ordering items, I click onto complete order
I am pulling up the name, address, credit card number and the items of the
last person who submitted an order. Does anyone know what causes this
problem? Is there a way to clear the form everytime the page is entered? I
noticed this is a problem on all browsers. Anyone have any information on
this problem? Thanks for the help.
