State, country, shipping and credit card information and all their
associated tags and form fields can be removed from the ord/checkout.html
page in the /pages/ subdirectory of your catalog directory. Note that this
is a fairly complex page with alot of inter-related minvend tags and code.
In particular, their is a javascript near the top which works with the
state field, and [set][/set] statements at the top and bottom of the file
which work with country and credit card info and toggle required fields.
If, for example, you remove the country field without modifying the code
that references it elsewhere in the page, you will get an error message
and/or erratic behavior when you try to process an order.
Plan to spend some time studying this page and referencing the docs before
you make any changes.

Adding new fields to the userdb is fairly easy, read the docs under THE
ORDER PROCESS, Customer Input Fields.
