This is a bug that I haven't fixed in 3.xx, for fear of side effects
at existing sites. I think it only affects catalogs the use the default
etc/report. Try setting in your checkout page:

    <INPUT TYPE=hidden NAME=mv_order_report VALUE="../etc/report">

It is fixed in 4.0.
