[mv] HTML bug fixes for MV4 beta 1 'basic' demo

Hello. 

Here are some bug HTML bug fixes for the MV4 beta 1 'basic' demo.

in the html/index.html file,

* Everywhere '__MVC_SERVERNAME__' appears, it needs to be prefixed with
 'http://', otherwise, it likely won't be recognized as a valid URL. 

*  the link to 'browse1.html' should go to 'browse.html' instead. 

* the link to 'feedback.html' is broken. It's not clear where this
should go.
