[mv] Got CyberCash Working, now fax_order fails!

Once again, I'm saying 'almost done'. I was about to open the store for
limited audience trials when I found that whether 'Credit Card' or 'Fax
Order' was checked on my checkout.html page, the full info went to
CyberCash.
I know that fax_order=1 and mv_payment=Send Check or Money Order Via Snail
Mail (we don't really accept faxes) because I added [value] tags on
failed.html; the values are correct and the info is still being sent to
CyberCash.
My first guess is that the test for credit card/not credit card order is
failing, but I'm not smart enough to figure out how. Here it is, from my
checkout.html page:

It looks good to me; I hope it doesn't to you. It may be relevant that my
catalog is based on the 3.12 sample, with code examples from 3.14 to make
CyberCash work thrown in.
I am running on Minivend 3.14
