Directive is "Catalog"
FIRST parameter is "simple" The catalog name. (More coffee)

Path to actual location of files

Watch your line wrap. AFAIK you can have any number of aliases after
the path to catalog files. They are all just alternate paths by which
that particular catalog can be called. If the catalog is called via one
of those aliases, it will generate return links via that alias (generally).
Search for "multiple entry points" in docs.
Catalog simple /var/mv/simple /Catalog/simple.cgi /Accounts/simple.cgi /Extranet/simple.cgi

No. The alias names are arbitrary. Your secure url is specified in catalog.cfg.
