[ic] minivend shipping?

Thank you for the reply.  I did change my criteria to [subtotal
noformat=1], but I still get $0 for all shipping.  Also it never asks
the customer which shipping method, so I feel like I am missing
something.  In catalog.cfg I have

CustomShipping  Yes

ValuesDefault    mv_shipmode       surf

Is this correct?
