[ic] username with blank on IC 4.8.2

Hi,
I have a problem of username in userdb with blank on my system using IC
4.8.2.
It is unclear what case or what operation to add new account, but this blank
username was created about once a month. Every time I remove username
with blank record and upload again.
I found the message below but I did not create by using the order desk.
