[ic] transactions and orderline adding fails

Dear List,

I recently upgraded to 4.8 from 4.6.  I'm getting this elusive error upon
checkout.

Neither the transactions or orderline db's gets updated, however both the
report and mail_receipt emails are sent.

Any ideas?
