[ic] Shipping by price - problems configuring, stays at zero

Barry, thanks for the reply.  I guess I'm confused about the shipping
options.  It must be that we're missing something obvious. In the
Admin/Shipping Configuration, we have set the FLATD method as follows:
   Total based on this criteria - Price
   Calculate based on this algorith - Standard shipping by price
with the resulting brackets as shown in the first post.

Should all other options in the shipping.asc file be deleted?  Is there
somewhere else to set a shipping flag?
