[ic] Customers getting charged... but no order in backend. Please help!

Today I've had five transactions that have charged the credit card but there is no sign of the order in the Interchange
backend. When the next order comes in there are missing order numbers. So to re-word... some orders are coming in just
fine but others are having this problem.

I've turned on debug and am waiting for the next one to happen.

My specs:
Interchange 5.2
Perl v5.8.4
MySQL Ver 12.22 Distrib 4.0.20, for pc-linux-gnu (i686)
Authorize.Net

This is what I find in my error log:

Has anyone seen this before? Any suggestions of what to do?
