Yep, happened to me lots of times. Sometimes it's been because the user
table already had a user with the username in it, and other times it's been
for reasons that I still am yet to deduce from the forensic evidence. It's
really quite frustrating. Sorry I haven't got anything more helpful to
suggest, but you're definitely not alone.
