I have a customer with 3 different stores up.
One store works fine in terms of placing orders.

Her 2 newest stores will not let you place an order.

The first one just keeps returning you to the checkout page even though
all the billing information is entered.

The second one generates the following message upon placing the order via
the checkout page:

	We need to have some fields entered to be able to process your order.
	Please go back and fill out the information. 

	Problem: 

	    Card: No format check routine for '(visa)' 

I cannot find any differences in the checkout.html pages (aside from stuff
like background color and font)

Does any one have any ideas?
