I would use item modifiers. They are fairly easy to setup, and are
automatically carried in the cart on an item-by-item basis, and don't have
to reference fields in any database if you don't want. The simple demo
implements this with size and color for a t-shirt; you could follow that
example for front and back designs, etc. Learn more in the docs under
PRODUCT PRICING:Item Attributes.
