[mv] Checkout returning to Checkout?

I'm using a varient of the basic checkout page. The problem is that when
I hit 'Place Order' the order is not processed and I am instead returned
to the checkout page. However, and this is odd, if I am logged in the
order is processed normally. How do I force the processing of an order,
or, how can I spoof a login (eg use a specific account each time)? 

Being that part of the ordering process is to write user information to
various databases I'm starting to think its a problem with not being
logged in. This is for one specific item (a gift certificate) so using a
bogus accoutn might not be a bad thing as I can track the number of
certificate ordered that way.
