[ic] Database INDEX - 2 Related Questions

>From my catalog.cfg:

Database            products     products.asc      TAB   
Database            products     INDEX             pageid
Database            categories   categories.asc    TAB   
Database            categories   INDEX             pageid

The products index is working. The Categories index doesn't (no
file generated, but no error in error.log). 

Does INDEX only work for the products file?

Is it necessary that the ItemNo field be included in any file 
	that is INDEXed?

Can't seem to find a definitive reference in either the site or
the mailing list archive.
