I found the solution:

In shipping.asc:

STD     UPS Canada Standard     weight  0       0       e Nothing to
ship!              {'ui_ship_type' => "UPSE:STD",'ups' =>
"1",'PriceDivide' => "1",}
STD     UPS Canada Standard     weight  0       150     f [ups-query
mode=STD weight="@@TOTAL@@"]               {'PriceDivide' => "1",}

Note: The code is STD, not UPSSTD.

Restart IC and then add STD to Canada's shipmodes in the states table.
