Well, there was some problem in the creation of the user account, 
obviously. Have you tried it yourself? A little interactive debugging could 
help. Perhaps grab that chunk of code that creates the user account from 
etc/log_transaction, put it on a test page and supply it with some test 
data and see what happens (that way you don't have to place an order to do 
a run-through).

I be inclined to think that it may have to do with etc/username.counter - 
maybe a permissions or file integrity issue.
