Try First Bank of Beverly Hills.  I talked with
several people about accounts and many were just
middle men, some working for FBBH.  FBBH negotiated
rates with me, and I use Signio as a gateway.  They
gave me my account number, and MID/TID numbers which I
plugged into Signio's software and everything worked
fine.

My business is new and small so my rates may not be
the best:

FBBH gets 2.3% cut + 0.30/transaction + $10/mo
statement fee (+a minimum of $20 for FBBH's "cut").
Signio gets $20/mo.

Setup was $200 total.  Including Signio's fees.
